<therapy practice name> is requesting your permission to use the IntelliSession software tool to help us with producing therapy notes. Therapy notes are normally done by hand and are used for medical record keeping and insurance purposes. IntelliSession produces therapy notes automatically for a session, resulting in more accurate and comprehensive notes and allowing your clinician to better serve you.

IntelliSession will run on your clinician's phone or computer during your session. Once the session is over, the session will be transcribed and a therapy note will be produced, with the audio and transcript being discarded immediately. Your clinician will use this therapy note for medical record keeping and insurance purposes. IntelliSession is HIPAA compliant and uses the best privacy and security standards.

By signing below, I am stating that I have read and understood the Informed Consent for permitting my clinician at <therapy practice name> to use IntelliSession to automatically produce therapy notes. I also understand that I may revoke my consent to use IntelliSession at any time, either verbally or in writing at any time.

Can I Use a Different Form Than the One Above?

Yes. The form above is only an example. You may use any consent form, as long as it clearly informs your client that their session data may be sent to IntelliSession and they provide informed consent.

When Should I Present This Form to My Client?

Most EHR (Electronic Health Record) systems manage onboarding paperwork for new clients. You can simply add the consent form above as an additional document for the client to review and sign.

Because using IntelliSession is optional, be sure to confirm that the client has signed this consent form before using IntelliSession during their session.

Is Client Consent Strictly Required?

If you plan to record the full therapy session (which we recommend), you must obtain the client’s consent in order to remain HIPAA compliant.

However, if you instead summarize or dictate session notes without including any personally identifiable information (such as names, home addresses, or details that could reasonably identify the individual), then client consent is not required.
